CLASSY FORMS

Please fill out the forms below. These forms for Daycare, Boarding, and Grooming are required prior to your pet's first stay with Classy Canine.

VACCINATION & TEMPERAMENT REQUIREMENTS

  • Well socialized, and friendly with no aggression issues

  • All pets must be at least 4 months of age

  • Puppies less than 6 months old are required to have 2 Bordetella vaccinations

  • All pets 6 months or older must be spayed or neutered

  • All vaccinations must be administered 72 hours prior to check in:

    • Rabies: Yearly or once every three years

    • Bordetella: Every 6 months

    • DHLPPC: Distemper/Parvo once per year
